Facility Placements

When the decision is made that an elder needs to be in a community care setting, we can assist the family in making the right match. Considerations need to be given to level of care needed, as well as the types of activities offered, round-the-clock nursing care, a stable staff, a facility in an acceptable location, and budget. We can offer a one-time consultation or an hourly rate for taking the family through every step of the decision process.

Services we provide

• Assistance with placement arrangements for alternative living (retirement, assisted living, or nursing home), whether short or long term
• Monitoring of any hospitalization stays (whether planned or unplanned)
• Monitoring of care in a retirement facility, assisted living facility, or nursing home after placement and providing ongoing reports to family and/or legal representative
